About the Role

This is a rewarding opportunity for a clinically experienced Registered Nurse who is passionate about supporting individuals living with complex disabilities and health conditions.

The Registered Nurse - Clinical Care Coordinator plays a pivotal role in supporting participants with complex care needs, including spinal cord injuries (SCI), acquired brain injuries (ABI), neurological conditions, and participants requiring high-intensity daily personal activities.

This position combines clinical oversight, stakeholder engagement, care coordination, and governance responsibilities to ensure participants receive safe, effective, and person-centered support. The Clinical Care Coordinator works collaboratively with participants, families, support workers, allied health professionals, case managers, and external stakeholders to develop and maintain high standards of clinical care and service delivery, in line with relevant practice standards and clinical governance requirements.

Key responsibilities include:

  • Conducting participant intake assessments, reviews, and ongoing site visits
  • Developing and reviewing individualised care plans
  • Supporting participants with SCI, ABI, neurological conditions, and high-intensity support needs
  • Building strong relationships with participants, families, and multidisciplinary teams
  • Completing clinical reports, risk assessments, and service reviews
  • Providing clinical guidance and training to support staff through initial onboarding and ongoing development
  • Monitoring quality, compliance, and participant outcomes
  • Contributing to continuous improvement and clinical governance activities

Skills & Experience

To be successful in this role, you will have:

  • Bachelor of Nursing
  • Current AHPRA registration as a Division 1 Registered Nurse
  • Qualifications or additional training in disability, rehabilitation, continence, respiratory care, PEG feeding, tracheostomy management, or positive behaviour support
  • NDIS Workers Screening Check
  • It is desirable that you have demonstrated experience in disability, community nursing, rehabilitation, complex care, or case management environments
  • Strong clinical experience supporting participants with spinal injuries, acquired brain injuries, neurological conditions, or high-intensity support needs
  • Strong understanding of NDIS Practice Standards and person-centred care principles
  • Exceptional communication, stakeholder engagement, and report-writing skills
  • Strong organisational and time management skills
  • Current Driver's Licence and willingness to travel for participant visits both in metro and regional locations
