The Opportunity

We are looking for a passionate and experienced Registered Nurse to join our team as a Clinical Care Coordinator. This is a dynamic, varied, and genuinely rewarding role that sits at the heart of what we do.

This is a work from home role with the flexibility and autonomy that comes with it. You will work across a caseload of participants with complex and high-acuity support needs, working closely with participants, their families, allied health professionals, support coordinators, and our internal teams to ensure the highest standard of care is delivered consistently and safely.

We have recently reshaped our Clinical Division, and this role is purpose-built to give you the focus and space to do your best clinical work.

What You Will Be Doing

• Managing a caseload of participants with complex care needs, ensuring care plans are current, person-centred, and reviewed on schedule
• Developing, implementing, and reviewing individualised care plans and risk assessments
• Conducting participant risk reviews and escalating concerns through appropriate clinical governance frameworks
• Liaising with participants, families, guardians, allied health professionals, and external providers
• Managing clinical incidents and documentation in line with NDIS Practice Standards and NIISQ requirements
• Acting as a clinical escalation point for support workers in line with UHC protocols
• Contributing to the continuous improvement of clinical practice and participant outcomes across the organisation

What We Are Looking For

• Registered Nurse (Division 1) with current AHPRA registration and NDIS Yellow Card
• Minimum 2-3 years post-registration experience, ideally with community, disability, or complex care exposure
• Strong communication and relationship-building skills across participants, families, and multidisciplinary teams
• Confidence managing clinical risk, incidents, and documentation
• A proactive, self-directed work style with the ability to manage competing priorities effectively
• Experience with clinical management systems - Lumary experience is an advantage
• Current driver's licence and willingness to travel to participant homes as required

Experience with spinal cord injury, acquired brain injury, neurological conditions, or other complex disability is highly regarded but not essential. We value clinical confidence and a willingness to learn above all else.
