The Role:
We are seeking an experienced and motivated Clinical Coordinator to join our Queensland team. In this pivotal role, you will oversee a caseload of participants, ensuring the delivery of high-quality, safe, and compliant care. You will act as the main point of contact for participants, families, and stakeholders, providing clinical oversight, coordinating care plans, and supporting our frontline staff.


Unidex Healthcare specialises in delivering high-quality care to participnts with complex needs, including those with spinal cord injuries and traumatic brain injuries. Our services encompass advanced clinical supports such as bowel management, catheter care, PEG feeding, mental health support, medication administration, and more.

This role would ideally suit a Registered Nurse that is looking for a career change, or for a Clinical Coordinator looking for a new challenge. The role will be primarily work from home, including access to a Satellite office in Brisbane.

We offer a flexible working arrangment including a combination of office-based and work-from-home options to support our team's work-life balance.

There will be ad hoc travel to other parts of Queensland, such as Rockhampton, Toowoomba, Mackay, Hervey Bay, and Cairns.


Key Responsibilities:

  • Lead and coordinate clinical services for a caseload of participants in line with their care plans.
  • Maintain and review participant documentation, care plans, risk assessments, and incident reports.
  • Provide guidance, support, and training to support workers and allied staff.
  • Liaise with participants, families, and external healthcare providers to ensure integrated care.
  • Monitor compliance with clinical governance frameworks, quality standards, and NDIS requirements.
  • Participate in multidisciplinary case reviews, audits, and service improvement initiatives.
  • Manage clinical escalations, incidents, and participant safety concerns.

Skills & Experiences:

  • Bachelor of Nursing
  • APHRA Registration
  • NDIS Worker Screening Check
  • Experience in care coordination, case management, or clinical oversight
  • Strong clinical knowledge in community or disability settings
  • Excellent communication, relationship-building, and stakeholder management skills
  • Strong attention to detail, organisational skills, and the ability to work autonomously
